| The first appointment will be a
free orthodontic evaluation. This appointment is designed to assess the need for
treatment. If treatment is needed we will discuss with you which type of treatment
is likely. Once we have decided to start treatment we will need to take special
measurements called records.

| At this appointment we will
take photographs, molds of your teeth (Impressions), a panoramic x-ray (Panorex) and a Cephalometric X-ray (Ceph). Records are needed to help us properly diagnose of your case.
This procedure generally takes about 30 to 45
minutes to perform. |

| Please check with your dentist
to see if he or she has a recent copy of your x-ray. If so you can have it
duplicated and bring it with you to your next appointment. |

Yes, there is a fee to have the records processed. |

The Consultation Appointment
The next step after records is the consultation
appointment. This is generally a question and answer session. At this appointment
you will be presented with a treatment plan. The treatment plan will consist
of the type of appliances or devices we plan to use to make the necessary
corrections. We will also discuss the anticipated treatment time, and the costs
associated with this phase of treatment.
We will try to answer as many of your questions as
possible. In order for us to start your treatment a modest down payment is generally
required.
Yes, we do offer payment plans. Payment plans are
designed to lessen the financial burden of treatment and make it more affordable. The
average treatment time is 18 months, so you can expect the payments to correspond to the
length of treatment. We offer multiple sibling discounts of %10 of those with two or
more children in treatment. |

stretch your payment out over longer periods of time. We will gladly help
process your insurance claims. We take many forms of payment such as cash, check,
credit card and we will try our best to make your orthodontic treatment
affordable. You can apply for Orthodontist Fee Plan simply
by visiting their web site and completing a credit application. In some cases you

| You are free to pay the entire amount at anytime
during treatment. If you decide to pay the entire amount in advance you will receive
a 10% cash courtesy that will be deducted from the entire amount. |
